Please complete the New Client Registration Form online prior to your first appointment here at the hospital.
New Patients
If you have any previous records for your pet, we prefer to receive them in advance so that we are ready for your pet’s first visit. If you have already provided us with the information to contact your previous clinic, we will have everything ready for your appointment. If you have records to share, please send them to us in advance of your pet’s appointment. Records should be sent to [email protected].
Surgical and Dental Procedures
For the health and well-being of all animals admitted as patients to our hospital, we recommend that pets are up to date on the following immunizations:
- Dogs: Rabies and Canine Distemper/Parvovirus (DHLPP)
- Cats: Rabies and Feline Distemper (FVRCP)
We do ask you to bring records of these vaccinations. If you are not sure about your pet’s current vaccination status, please feel free to ask one of our staff members to check your pet’s health records.
Certain medical procedures may require your pet to have fasted from food, water, or medications prior to arrival at our hospital for their procedure.
In general, no food or water after 10:00 p.m. the night before surgery. If your pet is a diabetic, please ask for special instructions regarding the 24-hour period before the procedure. Diabetic pets have special needs prior to a surgical procedure. “Pocket pets” should not be fasted.
